25 Cities San Diego is the local arm of a national initiative to end veteran and chronic homelessness initiated and funded by the U.S. Department of Veterans Affairs and supported by U.S. Department of Urban and Housing Development and the United States Interagency Council on Homelessness. In San Diego’s North County region, the 25 Cities San Diego initiative is coordinated by the Alliance for Regional Solutions, a collaboration that includes the partnership of over 30 community organizations. Interfaith Community Services leads on-the-ground efforts to establish a coordinated path to ending the cycle of homelessness in North County. Key public partners in this initiative also include the Department of Veterans Affairs, County of San Diego, as well as all nine North County municipalities.
What is the North County Initiative About?
The 25 Cities North County Initiative is designing a coordinated entry system that aligns with other regions in San Diego County and using it to end Veteran and chronic homelessness in our community. 25 Cities North County Initiative will prioritize data-driven, evidence-based solutions to rapidly connect as many homeless neighbors as possible to homelessness prevention services, permanent housing, health care, jobs, and other supportive services.

The 25 Cities North County Initiative had partnered will 211 to develop this portal to be used by the Housing Navigators/Case Managers and our homeless neighbors to efficiently link North County homeless individuals to necessary regional services that will assist in breaking down barriers to obtaining and maintaining permanent housing.
