Description of Service

Assessment site for the Opening Doors Coordinated Assessment and Housing Placement (CAHP) system administering the Vulnerability Index and
Service Prioritization Decision Assistance Tool (VI-SPDAT). The VI-SPDAT is an evidence based assessment tool that is used to determine the most appropriate housing intervention and enables providers to fairly and uniformly provide resources to those in need based on:

1. Level of need
2. Score on the assessment
3. Available housing resources

What happens after the assessment? After completing a VI-SPDAT, an individual or family may be assigned to a Housing Navigator based on scores, priorities, and room on staff caseload.

In order for homeless residents of the County to participate, they must complete a Vulnerability Index-Service Prioritization Decision Tool (VI-SPDAT).
