General Information

Description of Service

Provides daily outreach to homeless individuals throughout San Diego County including: Downtown, North Park, Uptown, City Heights, Hillcrest, National City and surrounding areas. The Outreach team is trained in crisis response and de-escalation. Also provides referrals to a variety of services including transportation and housing. Primarily focuses on placement for chronically homeless seniors, individuals with chronic and/or terminal illness, individuals with HIV/AIDS.

**IMPORTANT NOTE** Does NOT provide direct Transportation assistance to public.
