MyHealth served as the Convener and Data Aggregator for CMMI’s highly successful Oklahoma instance of the Comprehensive Primary Care initiative, which turned in annual Medicare savings of 5-7% per year for all for years of the program, and is currently serving as the Bridging Organization for the Oklahoma’s Route 66 Accountable Health Community. He will share strategies and best practices on improving and linking transitions of care through a diverse continuum including community based organizations.
