Peter Eckart, co-director, will discuss how DASH is connecting community collaborations across the country with resources to enhance their work.

Special Summit Highlights

Dr. David Kendrick

Dr. Kendrick will describe how MyHealth Access Network is serving as a public healthcare information utility for the state of Oklahoma to move healthcare and social needs data, and provide community-wide electronic coordination of referrals. MyHealth served as the Convener and Data Aggregator for CMMI’s highly successful Oklahoma instance of the Comprehensive Primary Care initiative, which turned in annual Medicare savings of 5-7% per year for all for years of the program, and is currently serving as the Bridging Organization for the Oklahoma’s Route 66 Accountable Health Community. He will share strategies and best practices on improving and linking transitions of care through a diverse continuum including community based organizations.

Annual Connections Luncheon

As part of the Community Information Exchange Summit’s closing celebration, 2-1-1 San Diego is hosting their Annual Connections Luncheon on Tuesday, April 17.

For more than a decade, the local tradition of the Annual Connections Luncheon has brought together hundreds of leaders from across all sectors of the region to celebrate community, partnerships and the mission that all people deserve access to resources to build and sustain healthy lives for themselves and their families. We are excited that this year the Connections Luncheon will provides the unique opportunity for San Diego’s leaders and the Community Information Exchange Summit attendees and speakers to join together to celebrate the power of community, collaboration and technology.
