COMMUNITY-CLINICAL LINKAGES BI-DIRECTIONAL REFERRAL PROJECT: DIABETES, PREDIABETES, HEART DISEASE, AND STROKE 

 

The Community-Clinical Linkages Bi-directional Referral Project offers opportunities for enhancing and strengthening bi-directional referrals between healthcare providers and lifestyle change programs under the National Diabetes Prevention Program (i.e., Diabetes Prevention Programs (DPPs)). Healthcare providers in San Diego County who are interested in referring clients/patients to lifestyle change programs, such as DPP, can utilize the resources on this webpage to learn more about the importance of lifestyle change programs and how to refer to a lifestyle change program in San Diego County.
