Family Health & Wellness

Our Client Care Specialists can help people in specific situations such as pregnant women, people experiencing homelessness, individuals with HIV/AIDS, and families connect with health and community resources specific to their needs. We do a comprehensive assessment that evaluates the whole person and we will advocate for you as you navigate the health and social service system, checking in on progress toward your goals and addressing any potential barriers to accessing services. You can connect with different services based on your individual needs:

  • Perinatal Care Network (PCN): A dedicated hotline for pregnant women in need of health insurance and/or primary care. To connect with the PCN line, call 1-800-675-222.
  • First 5 San Diego: Find resources for families with young children (the prenatal stage through five years of age) and pregnant women including family support, developmental screenings, oral health, and more. To learn more, call the First 5 warmline at 1-888-5-FIRST-5 (888-534-7785).
  • Housing Opportunities for Persons With AIDS (HOPWA): A program to help people with HIV/AIDS create a housing plan and find other social services to support overall stability. Call 2-1-1 to learn more.
  • SDHC Housing First: A program for people or households living in the City of San Diego who are at imminent risk of homelessness or have recently experienced homelessness and need rental assistance to remain in their current housing or to secure new housing. Call the SDHC Housing First line at (619) 578-7768 to see if you qualify and learn more.
